If you’ve been researching sclerotherapy, you’ve probably seen the same answer everywhere: you’ll need two to four sessions. That number isn’t wrong, but it omits some of the important details that affect your treatment planning.
At the Varicose Vein Center in Port Jefferson, NY, Dr. Jerry Ninia has spent 34 years treating vein conditions and has performed thousands of sclerotherapy sessions. His answer to “how many sessions will I need?” starts with a different question: what’s actually going on in your body?
Spider veins affect more than half of all women and are one of the most common reasons patients walk through our door. But no two cases are the same, and the right number of sessions depends on several factors that only a hands-on evaluation can determine.
The Short Answer: How Many Sessions Will I Need?
Most people do need two to four sclerotherapy sessions to see a satisfactory amount of improvement. Dr. Ninia has found from experience that most patients will see about 70% reduction of their spider veins after just one treatment.
However, this is a starting point. Some patients see excellent results after one or two visits, while others have more extensive vein patterns and will need to see Dr. Ninia several more times to achieve the results they want.
“Sclerotherapy isn’t always a one-and-done treatment. It may take time to get the results you want. The right number of sessions is different for everyone, and depends on what we find during your consultation.” —Dr. Ninia
What Determines How Many Sclerotherapy Sessions You’ll Need?
Your consultation is an important time of planning and evaluation. To determine how many sessions you’ll need, Dr. Ninia will look at a few things, such as:
The Type of Veins Being Treated
Spider veins are small, thin vessels visible just under the skin’s surface. They tend to respond quickly to sclerotherapy because they’re shallow and easy to access. A few isolated spider veins on one area of the leg might clear in one or two sessions.
Reticular veins are slightly larger feeder veins that often sit behind clusters of spider veins. Small varicose veins are deeper still. These take more time to treat and may require more sessions or a different type of treatment.
How Many Veins You’re Treating
A patient with a small cluster of spider veins on one calf is a very different case from someone with vein patterns across both legs. More surface area means more injection sites, and there’s a safe limit to how much sclerosant solution can be used in a single visit. Dr. Ninia treats as many veins as safely possible each session, but extensive cases simply require more appointments.
Your Body’s Healing Response
After a sclerotherapy injection, the treated vein collapses, and your body gradually reabsorbs it. How fast this happens varies from patient to patient. Factors like circulation and overall health can influence this timeline, as can skin type and activity levels. Some patients see veins begin to fade within a couple of weeks. For others, the full effect takes closer to six weeks per session.
This is one reason Dr. Ninia evaluates your progress at each follow-up before deciding on next steps. He always lets your body’s response determine the direction the treatment plan takes.
Whether There’s an Underlying Vein Issue
Sometimes the spider veins you see on the surface are a symptom of a deeper problem called venous insufficiency. When the valves inside your veins stop closing properly, it can start to allow blood to pool and create pressure that makes veins more visible and causes symptoms like aching and swelling.
If venous insufficiency is present, treating the surface veins isn’t going to be enough. The deeper issue needs to be addressed first, and Dr. Ninia will also likely recommend additional treatment steps, like using compression stockings and making sure you get enough movement every day. He uses diagnostic ultrasound to evaluate blood flow in your legs before building a treatment plan.
What to Expect from Your First Sclerotherapy Session to Your Last
Before Your First Session
Every treatment plan at the Varicose Vein Center begins with an examination. Dr. Ninia reviews your medical history, looks at your veins and may order a diagnostic ultrasound if he suspects deeper involvement. From there, he’ll walk you through what he recommends and why.
During a Session
A sclerotherapy session typically takes 15 to 30 minutes. Dr. Ninia injects a sclerosant solution directly into the veins using a very fine needle. The solution irritates the inner lining of the vein, which causes it to collapse and eventually be absorbed by your body. Sometimes the solution can sting a little as it is being injected, a good sign that the medication is working, but this is usually well tolerated by our patients.
Dr. Ninia personally performs every injection.
Between Sessions
Sessions are typically spaced three to six weeks apart. Your body needs time to absorb the treated veins, and Dr. Ninia needs to see how you respond before planning the next round. During this time, you can do your part by wearing compression stockings and staying active to support the healing process. You’ll also want to limit prolonged sun exposure on treated areas to reduce the chance of temporary pigmentation changes.
After Your Final Session
After completing a course of sclerotherapy, you should notice 70 to 90% improvement in your visible veins. Once absorbed, the specific spider veins and varicose veins that were treated will not come back. However, if there is deeper venous insufficiency, it’s important that you come back regularly for a check-up.
